Hospice care In Your Home
 

In-Home Hospice care enables patients to live out their lives surrounded by those they love while receiving quality care in their own homes.  Hospice care is provided in the privacy and comfort of your home or other community care facility.  We work in residential care, assisted living or skilled nursing facilities.  We bring the members of our professional hospice team to you at various times each week. 

 

 

 

 

Hospice care In The Hospice House

 

The Hospice House may be appropriate for some patients who require a higher level of care.  The Hospice House may also be appropriate for occasional short-term respite care.  

 

The Hospice House is a 10 bed residential facility for hospice patients staffed 24 hours a day.

 

The Hospice Care Team (which includes the patient's physician) will assist patients and families decide the best place to receive hospice services.
