Hospice Care

How services are provided by
each hospice team member
Hospice care
is personalized care. Personalized care means each patient has a
compassionate and knowledgeable Hospice Care Team
that works with them, their family and personal physician to address their
physical, spiritual and emotional needs. Our goal is to help
our patients live fully, completely and comfortably for as long as they
are able.
How services are provided by
each hospice team member

The
Patient's Physician
will continue
to direct
their patient's care.
*
The Hospice Medical Director
will work with the
patient's physician and oversee the hospice care.
*
A
Registered Nurse
will make regular visits to provide hands-on care,
teach family or
friends to be caregivers and manage pain and other
symptoms. A nurse is available for emergency response 24 hours a day,
every day. Families and patient's are not alone.
* Certified Home Health Aides
will provide personal care and assist with
activities of daily living.
* Social Workers
will
focus on the emotional, financial and social stresses
associated with terminal illness.
* A
Chaplain
will provide
pastoral care sensitive to each family and patient's
unique spiritual needs and desires.
* Volunteers will provide
companionship for patients, and necessary breaks
for caregivers.
* Bereavement Coordinators
will meet the needs of the family (children too)
for at least one year after their loved one's death.
